Case Manager -Family Support Worker

Overview:
CHL's Family Support and Stabilization Program, located in Leominster, provide services to families identified by the Department of Children and Families in North Central MA. These services are provided within the home, school and other community sites as indicated. The team consists of Clinicians, Family Support Workers and Youth Support Workers who provide intensive service and support. The program is designed to be flexible in hours and intensity, focused on the entire family. The team meets with families weekly to work on identified goals.
Responsibilities:
Provide home-based family support and connection to community resources for families of diverse socio-economic and ethnic backgrounds.
Work effectively with treatment team.
Maintain client records.
Assess client and family service needs and refer to appropriate services.
Attend trainings and staff meetings.
Qualifications:
Grade 5: Case Manager I: HSD, GED/HiSet or Associates degree required
Grade 6: Case Manager II: Bachelors degree required. Degree in social work, psychology or human services preferred.
Current valid US-issued driver's license and ability to provide registered and insured automobile for work-related purposes required.
Desire to serve others required.
Demonstrated strong work ethic, ability to work independently and ability to contribute to team objectives required.
Experience in social services and/or working with program's population preferred.
Some computer skills and desire to learn computer program used to maintain client records.
Must be able to pass a CORI background check

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
