- Provide case management services to residents - Conduct assessments to determine client needs and develop appropriate service plans - Coordinate and monitor client progress towards achieving goals - Collaborate with other service providers and community resources to ensure comprehensive support for clients - Advocate for clients and assist with accessing necessary services and resources - Maintain accurate and up-to-date case records and documentation
Experience:
- Minimum of 2 years of experience in case management or a related field - Knowledge of health insurance, crisis intervention, addiction counseling, motivational interviewing, behavioral health, supervising, and workforce development - Strong communication and interpersonal skills - Ability to work independently and as part of a team - Excellent organizational and time management skills - Proficient in using computer systems and software for documentation purposes This position requires a compassionate individual who is dedicated to helping others.
The successful candidate will have the ability to build rapport with clients from diverse backgrounds and provide support in a non-judgmental manner.
