As the Housing Coordinator, you will play a pivotal role in overseeing our Supportive Housing program, ensuring its compliance and effective reporting.
Essential Duties and
Responsibilities:
In this role, you will be responsible for a range of critical tasks, including:
The housing Case Manager will Manage the case management functions of the Supportive Housing program.
The housing Case Manager is responsible for conducting intake, assessment, and accurate data entry for program clients, utilizing the appropriate forms and assessment tools.
Collaborating with the director to complete monthly program reports and billing as required by funding sources and program contractors.
Providing case management services to assigned stabilization clients, fostering their progress and growth.
Building and maintaining strong relationships with landlords and property management teams to facilitate successful housing placements.
Assisting shelter clients in their housing search, including support with subsidized and market rate housing applications when necessary.
The Housing Case Manager will be working closely with community partners to ensure holistic support for program participants.
Upholding compliance standards of the Continuum of Care (CoC) regulations.
The Housing Case Manager will be Conducting regular team meetings to foster collaboration and synergy.
Keeping the supervisor informed of any concerns or potential challenges.
Attending relevant meetings and training sessions to stay updated on best practices.
Fulfilling additional duties as assigned by the Director of Housing & Workforce Development.
Required Qualifications and Skills:
To succeed in this role, the ideal candidate will possess the following qualifications and skills:
Associate's or Bachelor's degree in Human Services or a related field.
A minimum of two (2) years of experience at a comparable level, including staff supervision.
Demonstrated proficiency in case management techniques and strategies.
Strong written and verbal communication skills.
Excellent interpersonal abilities to connect with diverse individuals.
Empathetic understanding of the needs of a socio-economically diverse population.
Reliable access to a telephone.
