SCHEDULE:
Part-time or Full-time (day shifts, Monday-Friday) DUTIES:
Identifying individual social and emotional needs Developing and maintaining individualized care plans Participating in the planning of the resident's admission, community re-entry or transfer to another facility Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident.
REQUIREMENTS:
Previous experience working in long-term care preferred Must be a Licensed Social Worker in Massachusetts Bachelors degree in Sociology preferred Masters in Social Work/ (LMSW) Must be vaccinated against Covid Apply today! Job Types:
